This assessment was designed to assess your knowledge of elements of best practice in the area of community participation and social inclusion, in addition to the social model of disability and the impact of social devaluation on an individual’s quality of life. In addition, the assessment will allow you to demonstrate your problem solving, written and critical thinking skills.

1. This activity will require you to arrange and conduct an assessment meeting with a person with a disability. You should identify the appropriate forms of communication and means of enabling the disabled individual to express their wishes. The person’s strengths, interests, abilities and support requirements should also be identified. Once the assessment meeting has been completed you need to write a list of relevant groups and services for community inclusion. You also need to highlight the support requirements and means of overcoming any barriers that may be encountered. You will also need to write an account of the person’s strengths, interests, abilities and support requirements. Finally identify the forms of communication and strategies you used to enable the individual to express their wishes. Minimum 350 words

 

 

2. For this activity you are to consider a real or hypothetical instance of a person with a physical disability who wants to stay in their domestic home. You need to report on the modifications, types of support and devices which will enable the individual to enjoy the desired level of independence. You will also need to highlight the types of travel and transport arrangements which may be necessary to enable the individual to access the local community. Finally specify the means of evaluating and ensuring the ongoing success of your chosen strategies. Minimum 350 words
