Part IV – Implementation and Evaluation Plan Directions Phase D

Project topic: Burnout prevention in long-term care

There are many reasons why a change is not implemented, for example: funding is not available or there is disagreement among the key players or the logistics just do not come together. With this project most of you will not be able to put your plan into action point because time is short. To complete your Capstone, it is extremely important to have a detailed implementation and an evaluation plan in place.

1.List the specific steps you would take if you were planning to implement your project. What needs to be in place in order to implement your Capstone. List the specific, major steps that need to be taken in order for your Capstone to be implement. (Think “large picture here).

Examples:

Funding established, if needed.

Material reviewed by experts.

Work combined into one succinct proposal.

Write survey if using for evaluation.

2. Address resources needed. List each need, the rationale for it, and describe how you would obtain it. Be very specific here. For example: if you need to schedule a room, who do you contact to do so? If you need supplies, who approves the cost? Who orders the supplies? (Remember we do not need names here – just roles). Include at least 7 resources.

Examples:

Manager approval

Method of staff notification

Design notification or advertisement if using posters or flyers

Schedule space

Request equipment (laptop, projector, screen, table, etc)

3. Compose a professional email to your colleagues who will be affected by the implementation of this project. Include the following:

Description of the project

Rationale for the project

Benefits of the project

Acknowledgement of the impact on their work

Request support from your colleagues

4. Risks: Apply the systems thinking habit “Considers short-term, long-term and unintended consequences of actions” to your project. What could go wrong that could affect the success of this project? How will you foresee risks and plan to avoid them? This will be a minimum of two paragraphs, each containing at least 5 sentences.

5. Organizational culture: Are people ready to accept the change? Are there certain groups of people who may be more open to change? Others less open to change? What may motivate them to implement change? Refer to the diffusion of innovation in this response. This will be a minimum of two paragraphs, each containing at least 5 sentences.
